HOW TO CHANGE YOUR BENEFICIARY

1. Simply contact your financial institution to request a beneficiary designation form. You may also be able to log in to your account and update your beneficiaries online.

a. Please use our legal name: The Chautauqua County Humane Society Inc.

b. Include our tax identification number: 16-6000221

c. Please let us know that you have made us a beneficiary by emailing Holly Mullard at hmullard@chqhumane.org. We want to know about your generous gift to the animals!

2. Would you like CCHS to guide you through this process? We can help you with these changes so that minimal effort is required of you, and everything is done correctly as you intended.
